Rescues can be complex dynamic affairs often taking place in difficult locations and circumstances. It is inevitable that mistakes will occur or equipment will fail. Whilst we might feel embarrassed to admit our concerns or mistakes in public, it is essential for the team to know about them so we can improve our practices even if nothing actually went wrong on the day.
The form below allows for anonymous reporting of concerns, mistakes and near misses to the Team Leader, Chairman and Secretary. Unless you choose to add your name and email address, no identifying data will be sent with the message.
